It's pretty easy though - if you know how.
First of all you'll need a high-res pic, really high, so that you can make it smaler, when you're done designing. Thats comfortable, because you don't have to work that clean on the high-res pic, because after you've made it smaler, you won't see how "crappy" it orignaly looked.

Okay. When you have your pic, you have to cut all the logos and emblems first, and seperate them in a set or something.
After that you've to cut the jersey and desaturate it.
And thats nearly it.
You just need to cut the parts out now, that should be part of the "design". In my Case f.e. the parts on the arms and shoulders.
Then you'll go for layer -> layer style -> color overlay and choose your color.
Normal colors set on "multiply" or "overlay". For white, you take white on "linear dodge" at ~40% opacity.
Layer Overview could look like this f.e.:
